個人資料收集聲明 ▼
- 收集個人資料的目的
- 1. 你在本表格提供的個人資料,會供教育局用於以下一項或多項用途:
- a) 處理、核實及查證教師註冊及僱傭相關事宜,包括聘用、培訓及職業前途發展、薪酬及福利、員工關係、通訊和程序遵行;
- b) 處理、核實及查證向教育局提出的申請及向/由教育局作出的提名;
- c) 就上文(a)及/或(b)項所述事宜的處理、核實及查證,將個人資料與政府相關政策 局/部門資料庫進行核對;
- d) 將個人資料與教育局資料庫進行核對,以核實/更新教育局的記錄;
- e) 培訓及發展,包括發出計劃/活動邀請、處理發還課程費用申請、 評審提名、獎項和獎學金,以及監察達標進度;
- f) 處理及審核撥款/補助/津貼申請、發放撥款/補助/津貼,以及審計;
- g) 編製統計資料、研究及政府刊物;以及
- h) 執行規則及規例 [包括《教育條例》(香港法例第279章)及其附屬法例(例如《教育規例》、 《補助學校公積金規則》、《津貼學校公積金規則》)和《資助則例》]。
- 2. 你必須按本表格的要求及於本局處理本表格的過程中提供個人資料。假如你沒有提供該等個人資料,本局可能無法向教師提供相關的教育服務。
- 可獲轉移資料者
- 3. 你提供的個人資料會供教育局人員取閱。除此之外,本局亦可能會向下列各方或在下述情況轉移或披露該等個人資料:
- a) 政府其他政策局及部門,以用於上文第1段所述的用途;
- b) 與本表格相關的學校,以用於上文第1段所述的用途;
- c) 你曾就披露個人資料給予訂明同意;以及
- d) 根據適用於香港的法例或法庭命令授權或規定披露個人資料。
- 查閱個人資料
- 4. 你有權要求查閱及更正教育局所持有關於你的個人資料。如需查閱或更正個人資料, 請透過「查閱資料要求表格」(可於教育局網頁 主 頁 > 私 隱 政 策 (https://www.edb.gov.hk/en/privacy-policy/index.html) 下載)以書面提出,填妥表格後, 請郵寄到香港灣仔皇后大道東 213 號胡忠大廈 15 樓,保障資料主管人員收。
- 資料保留期
- 5. 個人資料將由教育局保存,直到其對於收集目的不再具有相關性為止。
- 保障資料原則
- 6. 當網絡欺凌涉及收集及使用個人資料,包括在公共領域內收集的個人資料,資料使用者(無論是個人或機構)須遵從私隱條例內保障資料原則的要求,當中保障資料第1及第3原則尤其重要:

- • 保障資料第1原則 — 此原則規定個人資料的收集必須與資料使用者的職能和活動有關 ,收集的資料屬必須及不超乎適度,收集的手法須合法及公平。
- • 保障資料第3原則 — 此原則限制個人資料使用於當初收集的目的或直接有關的用途上, 否則必須先獲得資料當事人明確和自願的同意。因此,將從公共登記冊、搜尋器及公共名冊之類的公共領域取得的個人資料用於起底或網絡欺凌,很可能違反保障資料第3原則。

Personal Information Collection Statement ▼
- Purpose of Collection
- 1. The personal data provided by you in this form will be used by the Education Bureau (EDB) for one or more of the following purposes:
- a) Activities relating to the processing, authentication and counter-checking of teacher registration and employment related matters including appointments, training and career developments, remuneration and benefits, staff relation, communications and compliance with procedures;
- b) Activities relating to the processing, authentication and counter-checking of applications made to and/or nominations made to/by the EDB;
- c) Activities relating to matching of the personal data with the database of relevant Government bureaux / departments in connection with the processing, authentication and counter-checking of the matters mentioned in (a) and/or (b) above;
- d) Activities relating to matching of the personal data within the database of the EDB for purposes of verifying / updating records of the EDB;
- e) Activities relating to training and development including invitation of participation in programmes/activities, applications for reimbursement of course fees, assessment of nominations, awards and scholarship, and monitoring of attainment progress;
- f) Activities relating to the processing and vetting of applications for, and disbursement of, funding / grants / subsidies, and conducting of audits;
- g) Activities relating to compilation of statistics, research and Government publications; and
- h) Activities relating to the administration and enforcement of rules and regulations including the Education Ordinance (Cap. 279), its subsidiary legislation (such as the Education Regulations and the Grant/Subsidized Schools Provident Fund Rules) and the Codes of Aid.
- 2. The provision of personal data required by this form is obligatory. In the event that you do not provide those personal data, we may not be able to provide relevant educational services to teachers.
- Classes of Transferees
- 3. The personal data you provide will be made available to persons working in the EDB. Apart from this, they may be transferred or disclosed to the parties or in the circumstances listed below:
- a) other Government bureau and departments for the purposes mentioned in paragraph 1 above;
- b) the school in which the form relates for the purposes mentioned in paragraph 1 above;
- c) where you have given your prescribed consent to such disclosure; and
- d) where such disclosure is authorised or required under the law or court order applicable to Hong Kong.
- Access to Personal Data
- 4. You have the right to request access to and correction of your personal data held by EDB. Request for access or correction of personal data should be made in writing in the Data Access Request Form, which is available on EDB website Home > Privacy Policy (https://www.edb.gov.hk/en/privacy-policy/index.html) and sent to the Controlling Officer (Data Protection), 15/F., Wu Chung House, 213 Queen's Road East, Wanchai, Hong Kong.
- Duration of Data Retention
- 5. The personal data will be kept by the EDB until it is no longer relevant for the purpose of its collection.
- Data Protection Principles
- 6. Where cyber-bullying engages the collection and use of personal data, the requirements of the Data Protection Principles (“DPPs”) in the Ordinance are relevant. DPPs 1 and 3 are particularly important:

- • DPP1 (Collection purpose and means) – This principle requires the data user to collect only the personal data for a purpose directly related to its function or activity; collect data as necessary and not excessively; and collect data by means which are lawful and fair.
- • DPP3 (Use) – This principle stipulates that unless the data subject has given explicit and voluntary consent, personal data shall only be used for the purpose for which it was originally collected or a directly related purpose. As such, cyber-bullying acts that involve the use of personal data of targeted persons collected from the public domain through channels such as public registers, search engines or public directories may constitute a contravention of DPP3.

- 1. Personal particulars of individual teachers are required in order to authenticate your identity every time you log in to this form. Every teacher has the responsibility to protect his/her own data and should follow the general instructions below:
- a. DO NOT divulge or share the login information unless strictly necessary. When sharing the information, it remains the user’s responsibility to ensure security of the data.
- b. DO NOT write login information in labels or obvious place.
- c. Be careful not to let other people see you enter login information.
- d. Immediately report to the Teacher Registration Team for any suspicious messages requesting you to divulge or share your login information.
- e. DO NOT obtain and use login information of other people by any means without authorisation.
- 2. The computers you use for accessing this form should not be installed with any Peer-toPeer (P2P) file sharing software. The computers should be properly installed with firewall, antivirus software, malicious code detection and repair software. All the security software should be activated at all times with the latest virus signature, malicious code definition and security patches. Regular system scan should also be performed to ensure that these computers are free from computer virus and malicious code.
- 3. Users must not log in to this form from public computers, e.g. those in cyber cafés, because the security level of these computers are unknown.
- 4. When connected to this form, password-enabled screen saver should be enabled and never leave the computer unattended.
